About the Service  

The Community Mental Health Team provides a comprehensive range of services designed to support recovery and promote sustainable community living for people experiencing mental illness.

The Community Mental Health CORE Team is a multidisciplinary team delivering specialist services to adults living in the community. The CORE Team sits within a broader Community Mental Health Network, which includes a specialist Aboriginal Mental Health Team, bilingual mental health workers, and Peer Support Workers, offering a culturally inclusive and person-centred approach to care. This position is located at Camperdown Community Health Centre, Royal Prince Alfred Hospital.

The Community Mental Health Team (CORE Team) is the mainstay of the Community Mental Health System. The multidisciplinary team includes Nurses, Occupational Therapists, Social Workers, Psychologists, Psychiatrists and Peer Support Workers. Together, we provide a range of clinical services, including intake, comprehensive assessment, care coordination, consultation, and tailored treatment for priority adult populations with mental illness.

About the Role

The Clinical Nurse Consultant (CNC) is integral to delivering recovery-oriented and high-quality Community Mental Health Care. The dynamic role is key responsibilities include:

  • Clozapine Coordination, management and education
  • Expert Care Coordination for consumers experiencing complex mental health needs living in the community
  • Provides leadership and innovation to ensure provision of best clinical practice within the multidisciplinary team
  • Contributes to the development and delivery of education programs 
  • Participates in formal processes for the strategic and operational planning for the clinical service

Ideal Candidate

  • Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and 5 years full time equivalent post registration experience.
  • Extensive understanding of speciality disorders and their current management.
  • Holds approved postgraduate qualifications relevant to the specialty or such other qualifications or experience deemed appropriate.
