Clinical Nurse Specialist Grade 2 - Integrated Care

Selection Criteria to be Addressed:

1. Current Registered Nurse Division 1 National Registration (AHPRA) with relevant post-registration qualifications and at least 3 years’ experience working in the speciality area of practice related to the management of clients with a chronic condition or those who have complex care needs, or such other qualifications or equivalent experience deemed appropriate by the public hospital or public health organisation.
2. Demonstrates a high level of confidence and clinical proficiency in the provision of care navigation, care coordination and health coaching
3. Demonstrated ability to effectively communicate and engage with clients, families/carers and the healthcare team to improve clinical service delivery, client outcomes and provide safe quality care that is culturally appropriate
4. Demonstrated ability to work as a member of a multidisciplinary team and to communicate effectively with a broad range of health professionals/ services and social services to facilitate the coordination of comprehensive and collaborative care
5. Demonstrated problem solving and decision-making abilities and high-level communication, liaison, negotiation, time management and report writing skills
6. Demonstrated ability to provide leadership in the provision and development of clinical practice, offer expert advice in the management of chronic and/or complex clients and has a demonstrated commitment to own professional development
7. Demonstrated knowledge of and ability to utilise information technology systems within a variety of applications and commitment to undertake training in electronic health record systems
8. Current valid Driver's Licence and willingness to use for work purposes. This may include driving long distances across the LHD
