What you'll be doing
This is an exciting opportunity to work across Western Sydney Local Health District supporting patients, families and clinical teams to enable, identify, plan and communicate goals of care, and to ensure an appropriate and multidisciplinary approach to End Of Life Care (EOLC) and Advanced Care Planning (ACP) is achieved.
Key responsibilities will be:
- Lead specialty nursing practice and service improvements
- Implement evidence-based clinical advancements for EOLC
- Mentor and support nursing staff and health professionals
- Advise on and represents clinical service development initiatives
- Collaborate across departments to enhance EOLC capabilities

Essential Criteria:
- Registration: Current Registration as a Registered Nurse with Australian Health Practitioner Regulation Agency (AHPRA)
Selection Criteria:
- Demonstrated at least three (3) years full time equivalent post registration experience in Palliative Care, Cancer Care Services, Advanced Nursing or clinical experience deemed appropriate by the employer
- Post graduate qualifications in Palliative Care, Cancer Care or Advanced Nursing
- Demonstrated skills and knowledge in case management, including care co-ordination for people living with a life limiting illness, using advanced clinical skills in patient/client assessment and care planning
- Demonstrated ability to provide leadership in the provision and development of clinical practice by utilisation of evidence based research with a demonstrated understanding of current NSW Policies and National and State Standards related to Palliative Care
- Demonstrated ability to communicate effectively with clients, carers and a diverse range of health care providers including those in government, non-government and community organisations
- Demonstrated ability to work within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion with an ability to identify and improve opportunities to enhance continuity of care between the acute sector, primary care providers and community based service
- Demonstrate ability to utilise information technology within a variety of applications
- Current Drivers Licence valid within Australia and willingness to use for work purposes
