About Us:

The Pathways to Community Living Initiative (PCLI) is a state-wide program dedicated to supporting consumers with enduring and serious mental illness who have experienced a long-term stay (greater than 12 months) in a mental health inpatient unit. Our focus is to assist individuals in re-establishing their lives in the community with appropriate and sustainable supports.


The Opportunity:

An exciting opportunity exists for a passionate and experienced Clinical Nurse Consultant to join the Community Transition Team within Mental Health Services. Based at the James Fletcher Hospital Campus in Newcastle, you will be a valued member of a multidisciplinary team.



In this role, you will:

  • Collaborate with inpatient and community services to assess and facilitate smooth transitions from hospital to community settings.
  • Provide direct clinical interventions to consumers.
  • Offer expert clinical consultancy and professional leadership to internal and external stakeholders across the Local Health District (LHD).
  • Consult into the development of individual transition plans and care pathways to the community and enhance the transition process between inpatient and community settings to support optimal outcomes and experience for the consumer, families/carers, mental health clinicians and CMO partners.  

The role is supported by the Team Manager, PCLI Program Manager and mental health nursing leadership. The role will continue to develop and evolve over time in line with developments within PCLI. You will also be part of a state-wide network of PCLI clinicians.


About you:

The successful applicant will be able to demonstrate extensive experience and knowledge of complex mental health presentations, nursing leadership and highly developed communication and interpersonal skills. You will also have the ability to communicate clinical concepts and information to non-clinical audiences and provide advice on clinical issues within a recovery and rehabilitation context.


Essential Requirements:

  • Registered Nurse with current Authority to Practice with AHPRA who has at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the field of Mental Health.
  • Postgraduate nursing qualifications in Mental Health.
  • Eligible to drive in NSW with a current unrestricted NSW Drivers Licence
