We are looking for an experienced Clinical Nurse Consultant to join a dynamic and dedicated team.

Your new role...
The Clinical Nurse Consultant position will be based at Canterbury Hospital.

The Clinical Nurse Consultant (CNC) provides expert clinical advice to patients, carers and other health care professionals within a defined specialty through the delivery of specialist consultant service which:
  • Develops implements and evaluates care management plans for patients with complex health needs
  • Provides leadership in the ongoing development of clinical practice
  • Initiates and utilises findings of research in the provision of clinical services.
  • Contributes to the development and delivery of specialty related education programs
  • Participates in formal processes for the strategic and operational planning for the clinical service.

The “Community Older People’s Mental Health Team” (OPMH Team) is a specialty service of the community mental health system.

OPMH Team provide a range of clinical services including intake, assessment, consultation, treatment and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care and short term care coordination.

Consumers under the care of OPMH Teams will either have:
  • Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months
  • A period of treatment as recommended by evidence based practices

To Be Considered For This Role You Must Have:

A full NSW drivers license (P2 Acceptable)

The ideal candidate will have...

  • Current nursing registration with the Nursing and Midwifery Board of Australia (AHPRA) and at least 5 years full time equivalent post registration experience.
  • Approved postgraduate qualifications relevant to the specialty or such other qualifications or experience deemed appropriate.
  • Computer skills including word processing, presentation, spread sheet applications and clinical information systems.
