About the Opportunity
We are seeking an adaptable and compassionate individual to join our team as the Clinical Needs Assessor.
If you have a passion for working with older persons, possess strong teamwork and communication skills, and have previously worked in the aged care sector, we encourage you to apply.
In this role, you will conduct complex aged care assessments using the Integrated Assessment Tool (IAT). These assessments will address the medical, physical, cognitive, psychosocial, cultural, environmental and social aspects of older people referred for assessment via My Aged Care.
What You'll be Doing
The clinical assessor formulates a support plan that best meets the needs of the older person and their carer within the resources available.
Works within the guidelines set by the Single Aged Care Assessment System Contract and Statement of requirements, the My Aged Care Assessment Manual 2024 and the Aged Care Act 1997 and any amendments.

- Hold tertiary qualifications as per clinical discipline (nursing, occupational therapy, physiotherapy, social work) with full AHPRA registration or part of or eligible to be part of a relevant professional association.
- Willingness to undertake the Transition Training (during the Program Transition Period). Completion of training requirements as specified under the My Aged Care Workforce Learning Strategy 2023.
- Demonstrated a minimum of one year experience in delivering services in aged care settings and/or to aged persons (such as geriatric evaluation, rehabilitation, palliative care, community nursing), including to people living with dementia.
- Demonstrated understanding/experience of the provision of community-based support services to the aged reflecting wellness/reablement approach to clients, including assessment, care planning, monitoring, and evaluation processes.
- Previous experience performing in a KPI environment to ensure good quality client outcomes.
- Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
