  1. Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) with relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate for the position e.g. Substantial and current experience in Complex Care Coordination, using advanced clinical skills in patient assessment and care planning.
  2. Experience in practice that promotes self-management support, behaviour  change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.
  3. Ability to identify and improve opportunities to enhance continuity of care between acute hospitals, primary care providers and community-based services and a comprehensive knowledge of primary health and community based services.
