Join our dynamic Inpatient Social Work team at CCLHD, as a Social Worker NSW Health Professional Award (HPA) Level 3 Team Leader, specialising in Geriatric services at Gosford Hospital. Here, you'll play a crucial role in supporting patients and their families in their emotional and psychosocial response to older patients experiencing illness diagnosis, disability and treatment, supporting patient and families/carers as they navigate their journey, including interfacing with NSW Civil and Administration Tribunal (NCAT) Guardianship processes and/or transitioning to residential aged care facilities with optimal care and support.
About the Role
Located at Gosford Hospital, you will be working with a highly skilled and motivated multi-disciplinary team who are dedicated to patient centred and trauma informed care. The role provides social work support to patients of varying ages, who have experienced a neurological event.
As a Social Work Team Leader in Geriatrics Services, you'll draw upon your extensive clinical expertise to deliver top-tier care within a multidisciplinary framework. Operating autonomously within our team, you'll provide senior leadership to fellow inpatient social workers, proactive service development, education, and supervision. Your role encompasses comprehensive assessment, crisis intervention, and counselling, alongside facilitating seamless transitions to community services and support networks.

About You
- You hold Tertiary qualifications in Social Work, with eligibility for membership of the Australian Association of Social Workers (AASW) (overseas qualifications must supply the AASW Overseas Assessment number). A demonstrated understanding of the current AASW Practice Standards as they relate to the Social Work Team Leader position.
- Demonstrated social work experience in the Geriatrics area and in providing psychosocial assessment and crisis management, within person centred, trauma informed and brief intervention models, and well-developed understanding of the psychosocial issues that arise for patients/clients and their families during chronic and acute illness and trauma.
- You have demonstrated team management and leadership experience of a clinical team providing supervision to staff and students, effectively managing staff professional development and performance and the building of a mutually supportive team.
