As a Palliative Care Nurse in our Community Nursing team, you’ll have the
opportunity to make a genuine impact in the lives of individuals and their families during some of the most challenging times they face.


About the Role


As a Community Palliative Care Nurse, you’ll provide exceptional, patient-centered care to individuals within their home settings. Working closely with a multidisciplinary team, you’ll ensure the comfort and dignity of patients at the end of life, providing physical and emotional support to both patients and their families. Your role will involve effective communication, coordination of services, and continuous care management. With a focus on evidence-based practice and quality improvement, you’ll be able to influence positive outcomes for your patients, making a real difference in their journey.   Position Description


About
 You

  • Current AHPRA registration as a Registered Nurse with an Australian Driver's Licence.
  • Extensive experience in palliative care nursing within a clinical setting, with a commitment to providing compassionate and high-quality care.
  • Strong focus on quality improvement, practice development, and evidence-based practice.
  • Demonstrated ability to communicate effectively through written, verbal, and interpersonal skills, using information technology where necessary.
  • Proven ability to work effectively as part of a multidisciplinary healthcare team and independently when required.
  • Personal attributes including resilience, integrity, and accountability that contribute to a positive and supportive workplace.
  • A genuine desire to make a meaningful impact in palliative care and the lives of patients and their families.
