Role title:  Assistant in Nursing (Year 1 - 4)

Role type:  Max term contract part time (32 hours per week until June 2026)

Department and location: Transitional Aged Care Program (TACP), St Vincent’s Hospital, Darlinghurst

Salary:  $51,844.63 - $56,885.08 (pro-rata) + super.

About the role

TACP provides a package of services in the clients home for up to twelve weeks duration to clients following discharge from hospital and who have potential to manage at home with rehabilitation and support services.

  • Provide support to the nursing team, both Enrolled and Registered, in the delivery of nursing care in an acute, sub-acute care and community environment.

  • Tasks may include gathering of equipment, simple wound dressing, assisting clients with their Home Exercise Programs (HEP), escorting/community access, showering and bathing clients and light domestic assistance.

  • Document all client responses to direct care given, including progress notes and clinical records in accordance with the plan of care and organisational procedures.

  • Assist in the delivery of therapy and support services for clients of the TACP, working with a large multidisciplinary team to provide slow stream rehabilitation to clients who have recently discharged from hospital.

  • The purpose of this intervention is to improve the client’s physical function, avoid further hospital presentations and to maintain their daily living skills.

  • This position is a part-time position, 4 days per week, with the service operating Monday – Friday. Occasional weekend or public holiday work may be required.

About you

  • Relevant Certificate III qualification or have completed one (1) or more year training of a Bachelor of Nursing degree.

  • Current unrestricted NSW drivers' license – Class C or above.

  • Demonstrated experience working with older people in a community/rehabilitation setting.

  • Demonstrated ability to work independently, under delegation, and the ability to collaborate within an interdisciplinary team.

  • Demonstrated effective interpersonal, written and verbal communication skills.
