About the Role
Join the Transitional Aged Care Program (TACP) team at Armidale Community Health Service and play a vital role in supporting aged care clients during their recovery journey post-hospital discharge.
As an Assistant in Nursing, you will work closely with Allied Health and Nursing professionals to provide person-centred, enablement-focused care to clients in their own homes. This may include:

Assisting with personal care and light domestic duties

Supporting therapy activities and exercise routines
-
Transporting clients to appointments, social events, and group programs
-
Helping clients reach their functional and independence goals
Your role will be varied, autonomous, and deeply rewarding.
