About the Opportunity
The Aboriginal Health Worker will be based in the SWSLHD Out of Home Care (OOHC) Team.

  • Fun, friendly, very supportive team
  • Flexible Work Practice available – including working from home
  • Home visiting model completing health assessments with a range of clients meaning every day is different
  • Work with infants, children and young people aged 0-17 years and their families supporting cultural wellbeing and connection
  • Multidisciplinary team comprised of Child and Family Health Nurses and Social Workers
  • Working along side DCJ, NGOs, Youth Health, PARVAN, Child and Family Health, Paediatric, Mental health and allied health teams

The Aboriginal Health Worker will work closely with the multidisciplinary team to provide culturally responsive assessment, care planning and support coordination. The Aboriginal Health Worker will also be responsible for providing advice and education to the team to improve access to services for Aboriginal young people and their families/carers.

As a part of the SWSLHD Out of Home Care Team, you will be supported by an Aboriginal PARVAN Lead and have access to a wide range of cultural supports across the service including cultural supervision, SWSLHD Aboriginal Health Unit and a large workforce of Aboriginal colleagues. Where you’ll be working:
SWSLHD operates its services on the lands of the Dharawal, Darug and Gundungurra peoples and has a rich history of working alongside local Aboriginal organisations and programs.

The position has an office base in Ingleburn. The team services community across the South West Sydney area and travel across the district will be required.

  4. Applicants must possess an Australian C-Class driver's license (including P1 or P2). Those who hold a C class license (P1 or P2 included) from a state other than NSW must obtain a NSW C class (P1 or P2 included) license within three months of appointment. Learner's licenses are not permitted.
