In your role as Administrative Officer Level 3 - Discharge Patient Flow Concierge (DPFC) you will provide a range of administrative and clerical support services to enable improved patient experience and Access and Flow Key Performance Indicators. 

 

You will work as part of the patient flow team to support clinical staff with the planning, communication and follow-up of patients following discharge from hospital.

 

You will also support the Patient Flow Manager with care coordination processes, data collection and recordkeeping

Shoalhaven Hospital, located on Yuin Country, is the main acute care hospital for the Shoalhaven region, providing emergency care, medical, surgical and orthopaedic services. The hospital has an Intensive Care Unit focusing on high dependency and coronary patients, Children’s Ward and Maternity Unit, as well as a purpose-built regional Cancer Care Centre and sub-acute Mental Health Unit.
