About the role
The Transitional Nurse Practitioner (TNP) is a Registered Nurse authorised to function autonomously and collaboratively in an advanced and extended clinical role within a service.
The role will manage a broad range of acute, chronic and complex presentations in hospital, primary health and community settings in a multidisciplinary setting and with collaboration with specialist and community-based services.
As the TNP you will contribute to the advancement of quality health systems, within Integrated Care, to reduce avoidable hospital admission and promote early discharge from hospital. Through the provision of comprehensive systematic patient assessment and determination of appropriate diagnostic interventions necessary to deliver advanced and holistic management of patients attending the Orange Hospital Ambulatory Care unit, nursing homes or patient's homes, you will support the continuum of care across the inpatient and community setting and act as an expert on service availability promoting safe and effective patient care.
In this position you will also facilitate multidisciplinary care by referring to relevant disciplines for issues outside the scope of the TNP. It’s not always easy, but it will always be extremely rewarding.
