Allied Health Professional (Level 3): $2,154.06 - $2,223.86

Macquarie Hospital: The Lodge, TACP
What you'll be doing
TACP Client case finding and liaison with NSLHD Public acute and sub-acute hospitals
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Recognised degree in a health-related field (Nursing or Allied Health Professional) with current professional registration or eligibility for membership of relevant professional body as relevant.
- Demonstrated recent post-graduate clinical experience in aged care settings.
- Demonstrated skills in the coordinated assessment and case management of older persons with complex care needs.
- Demonstrated experience in team leadership, with the capacity to interact with others in a personable and professional manner and personally resolve conflicts when they arise.
- Demonstrated effective communication, interpersonal, and time management skills to appropriately engage with patients/consumers, carers and the multidisciplinary health care team to identify and ensure responsiveness of care needs.
