Where you'll be working:
Based at Muswellbrook Community Health Service, you’ll be part of the Hunter Valley Transitional Aged Care Program (TACP)—a multidisciplinary team supporting older clients to regain function and independence after hospital discharge. Serving the Upper Hunter and surrounding communities, TACP offers person-centred, home-based care with a focus on enablement and dignity. You’ll enjoy a supportive team environment with Allied Health and Nursing professionals dedicated to improving quality of life. The service operates 8:00am–4:30pm, seven days a week, with this role offering five 8-hour shifts per fortnight.
About the role:
As an Assistant in Nursing (AIN) in the TACP team, you’ll deliver compassionate, practical support to clients in their homes. You’ll help people maintain independence and reach their recovery goals through respectful, person-centred care.
Key responsibilities include:
- Assisting with personal care and daily living activities
- Providing simple wound care and medication prompts
- Supporting home-based exercise programs developed by physiotherapists
- Helping with light domestic duties, grocery shopping, and transport to appointments
You’ll work independently with a work vehicle and mobile phone provided, while being backed by a responsive, collaborative team whenever needed.
About you:
You are a self-motivated, compassionate individual who enjoys one-on-one care and thrives when working independently in a community setting.
You’ll bring:
- A Certificate III in Assistant in Nursing
- Excellent communication and time management skills
- Flexibility, initiative, and a strong team ethos
- A genuine commitment to person-centred care and helping others live independently
Requirements:
- Certificate III Assistant in Nursing
- Current NSW driver’s licence with the ability and willingness to travel to clients’ homes
- Availability to work five 8-hour shifts per fortnight across the 7-day service model
