Social Work Team Leader, Chronic and Complex Care Team
- Social Work degree and eligibility for the Australian Association of Social Workers (AASW) with extensive experience at a Level 2 or above with a demonstrated understanding of the psychosocial impacts of living with chronic health conditions and hospitalisation.
- Demonstrated advanced clinical skills in comprehensive biopsychosocial assessments, counselling and therapeutic interventions, complex clinical casework, advocacy and discharge planning.
- Demonstrated ability to provide guidance, leadership and management of a Social Work team.
- Demonstrated advanced communication skills (both written and oral) and demonstrated competence in the use of electronic information systems
- Excellent negotiation, complex decision making and advocacy skills including the ability to interact constructively and collaboratively with a diverse range of stakeholders.
- Demonstrated ability to prioritise work and manage stressful situations; ability to work unsupervised and apply advanced reasoning skills and independent professional judgement when dealing with situations of a novel or critical nature across a team.
- Demonstrated understanding of and commitment to quality processes and the application of evidenced based practice within Social Work.
