What you'll be doing
Join a dynamic healthcare team as the Service Coordinator, Motor Neurone Disease (MND), where you’ll lead and coordinate specialised MND care across Western Sydney and Nepean Blue Mountains. This key role ensures seamless clinical operations and patient-centred care.
- Act as the main contact for all MND patients, coordinating specialist care and services.
- Provide expert advice on MND to patients, carers, clinicians and service providers.
- Lead service delivery, quality improvement, education and data initiatives.
- Manage patient care pathways, including triage, planning, admissions and MDT reviews.
- Collaborate with stakeholders to enhance care systems, communication and outcomes.

Essential requirements:
Qualification: Relevant qualification including Bachelor of Occupational Therapy, Speech Pathology, Social Work, Physiotherapy or Nutrition and Dietetics
Registration: Eligible for professional registration within their respective discipline and if applicable, registration with AHPRA.
Selection Criteria:
- Tertiary qualified Allied Health Professional in Social Work, Speech Pathology, Physiotherapy or Occupational Therapy.
- Able to demonstrate extensive knowledge and experience managing complex clinical situations for people with progressive neurological diseases
- Demonstrated experience in successfully coordinating a multidisciplinary team or service with demonstrated ability to engage and consult with relevant stakeholders.
- Demonstrated ability to work autonomously, apply independent professional judgement and advanced clinical reasoning skills to a range of clinical situations.
- Demonstrated ability to manage competing priorities and deliver outcomes to agreed deadlines.
- Demonstrated well-developed and effective communication and negotiation skills including the ability to produce high quality documents and correspondence.
- Demonstrated experience in leading and evaluating quality improvement initiatives and /or service development.
- Current Drivers licence valid within NSW and ability to travel as required across the local health district.
