About the role
Dubbo Health service Dialysis Unit is a Level 5 Renal Service providing services in Dubbo whilst supporting satellite units (Mudgee, Nyngan) and facility-based units (Brewarrina, Bourke, Cobar, Coonabarabran, Coonamble, Gilgandra and Wellington). In addition to this Dubbo Health Service offers vascular surgery services, endovascular surgery access and renal transplant workups for patients with chronic kidney disease.
The Nurse Practitioner / Transitional Nurse Practitioner will work alongside specialist Nephrologists to provide timely assessment, diagnosis, intervention, education, and access to treatment for patients with identified renal disease in Dubbo, Central and Northern Sectors of Western NSW LHD.
Within our Northern & Central Sectors, 48% of patients identify as Aboriginal or Torres Strait Islander. The aim of this position is improved patient experience through access to care, continuity of care and access to specialised clinicians.
