Pathways to Community Living Initiative (PCLI) is a coordinated state-wide approach to supporting people with enduring and serious mental illness who have been in hospital for more than twelve months and or are likely to experience difficulty with discharge solutions, to re-establish their lives in the community.

Illawarra Shoalhaven Local Health District is committed to building a professional workforce that supports consumers with a severe and persistent mental illness on their recovery journey. The role of the Senior Clinician is to provide district-level evidence-based care that supports safe transitions of consumers into care in a community setting (or appropriate inpatient setting) based on the individual consumer’s strengths, needs and wishes. These services are to be least restrictive, collaborative, nominated carers and family responsive and inclusive, rehabilitative, encourages self-advocacy, facilitates empowerment and capacity for optimal independent function.


A primary goal of this position is to work collaboratively with existing inpatient and community teams, consumers, carers and other key stakeholders in the coordination of day-to-day work practices for teams working with PCLI Consumers, by supporting the assessment and progression of safe transitions of consumers into care in a community setting while promoting effective participation in mental health treatment, support and recovery.

This role will carry a clinical caseload, championing practice improvement through mentorship and supervision, to ensure the delivery of evidence-based, strengths focused, recovery orientated assessment and treatment.

·         Applicable vaccination requirements for a Category A role.

·         Working With Children Check (WWCC)

·         Current AHPRA Registration or Social Work degree and eligibility for the Australian Association of Social Workers (AASW)

·         Registered Nurses must hold relevant post-registration qualification and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate.

·         Occupational Therapists, Social Work and Psychologists must be able to demonstrate extensive specialist knowledge in the area of mental health.

·         Valid NSW Drivers Licence
