About the role
This includes assisting people to access specialist health care and ensure they are supported in improving their health and wellbeing outcomes. The HHOT will provide services to people who are chronically homeless with complex needs and diverse value systems, and who do not access standard health services. While there is a strong focus on mental health and drug and alcohol issues, general physical health issues will be within the scope of this team. This team will work very closely with the non-government services and other local and state government services that are involved in providing services to this clientele.
The Clinical Nurse Specialist Grade 2 (CNS2) coordinates and delivers high quality nursing care to patients/clients in accordance with the Nursing and Midwifery Board of Australia’s National Standards of Practice for Registered Nurses, criteria for CNS2 as outlined in the Public Hospital Nurses and Midwives (State) Award and the Policies and Procedures of the Ministry of Health (MoH) and Northern NSW Local Health District (NNSWLHD).
The Northern NSW Local Health District (NNSWLHD) Mental Health CNS2 is primarily responsible for the efficient and effective coordination of patient care to homeless people who have complex mental health issues and facilitate safe, evidence based, person centred nursing care in line with the standards, policies and procedures of the Ministry of Health and other statutory authorities.
