About Your Role

This position will be responsible for educating, enabling and supporting team members to identify and communicate goals of care and ensure a multidisciplinary approach to end of life care (EOLC) and Advanced Care Planning (ACP).

This position will build on the current excellent work and educational initiatives, strengthening the development of sustainable educational programs in palliative care. This role aligns with the standards set by HETI, integrating clinical expertise, ensuring high-quality patient care in this specialised field.

Key activities and contribution to the organisation include;

  • Maximise Advanced Care Directive awareness and education benefiting both inpatient, community, and MLHD aged care facilities
  • Embedding advanced care discussions into business as usual for nursing, aged care, allied health, and medical teams and our MLHD community members
  • Enhance education with new knowledge, technology, and practices for world class workforce.
  • Customise HETI learning pathways to MLHD and improve training networks for better access, engagement, and optimal performance.
