About the role:   

Join a high performing multidisciplinary team, where you will provide high level clinical assessments. The role of the Clinical Aged Care Needs Assessor is to utilise the Integrated Assessment Tool to determine the care needs of frail older people in any setting and facilitate access to available care services appropriate to their care needs.

As the Clinical Aged Care Needs Assessor, you will be responsible for:

  • Undertaking comprehensive Aged Care clinical assessments.
     

  • Working collaboratively within a multidisciplinary team to optimise the health care and wellbeing of each client and their loved ones.
     

  • Coordinate and facilitate access to the most appropriate type and classification of aged care and other support services that best meets the Client's current, assessed care needs.
     

  • Supporting complex cases with a variety of challenges and considerations to navigate

About You   

Our ideal candidate will demonstrate:   

  • High level of professionalism with ability to work independently and as part of a large team
  • Exemplary communication skills, both written and verbal
  • Minimum 1 year experience in Australia directly delivering services in aged care settings and/or to aged persons.
  • Tertiary qualification in a health-related discipline directly related to health, aged care, or related specialist area (nursing, occupational therapy, social work, physiotherapy, psychology), with full AHPRA registration or eligibility for membership of the Australian Association of Social Work.
