Our Occupational Therapy Services within the Transitional Aged Care Program (TACP) are designed to help clients transition smoothly from hospital to home, ensuring optimal recovery and long-term well-being.

Whether through our Transpac community program or in our Transitional Care Unit, our dedicated team of professionals works closely with clients and their families to provide personalized, goal-focused care that addresses individual needs.

Our Services Include:

  • Restorative Therapy: Assisting clients in completing the recovery process and regaining functional independence.
  • Functional Optimization: Enhancing daily living skills to improve quality of life and independence.
  • Long-Term Care Planning: Helping clients access and finalize ongoing care arrangements for sustainable health outcomes.

An Eligibility List (E-List) or talent pool may be created from this recruitment episode to assist in filling future permanent, temporary, or casual full-time and part-time positions. 

About You

  • You have have a current Occupational Therapy registration with AHPRA.
  • You have demonstrated ability to analyse problems, set priorities and apply a range of options to develop solutions.
  • You have demonstrated ability to work as a member of a multidisciplinary team.
