At Central Coast Local Health District (CCLHD) we provide specialised Social Work and Case Management services within the Transitional Aged Care Program (TACP) to support individuals and their families during critical transitions from hospital to home. Our experienced team works across both community-based (Transpac) and residential (Transitional Care Unit) services, ensuring that every client receives the care and support needed to achieve optimal health outcomes.

Key Services We Offer:

  • Adjustment to Illness and Disability: Our Social Work team helps clients and their families adapt to the challenges of illness or disability with professional guidance and support.
  • Client and Carer Empowerment: We provide practical assistance and emotional support to enable clients and their carers to navigate the care process with confidence.
  • Long-Term Care Planning: We assist clients in accessing and finalizing suitable long-term care arrangements, ensuring a smooth transition to ongoing care services.

An Eligibility List (E-List) or talent pool may be created from this recruitment episode to assist in filling future permanent, temporary, or casual full-time and part-time positions. 

About You

  • You have a degree in Social Work from an accredited course providing eligibility for membership of the Australian Association of Social Workers and a commitment to maintaining a professional skill and knowledge base.
  • You have a current unrestricted Australian Drivers Licence.
  • You have demonstrated commitment to professional development, evidence based practice and continuous improvement.
