Make a meaningful impact as a Clinical Nurse Specialist, coordinating care for older patients with complex needs across acute and emergency settings. In this role, you’ll conduct comprehensive assessments, facilitate timely discharge planning, and connect patients and their families to essential aged care services, including transitional care, home care packages, and residential support. Collaborate with multidisciplinary teams, educate staff on geriatric care, and advocate for equitable access to services for diverse communities.
