About the Role

Sydney Local Health District’s Community Health Centres have a long and proud history of delivering care in the community including prevention, early intervention, assessment, treatment, health maintenance and continuing care services designed to improve and support the health and wellbeing of individuals and communities.This role will be based across SLHD. 

 
The AOW will form part of a collaborative professional team providing culturally appropriate support and care in partnership with the Chronic Care Coordinators to people with a chronic disease. Working with the Chronic Care Coordinators the role will establish links with Aboriginal communities and with  individuals, identify barriers to managing health, and encourage and support access to health services.

Ideal Candidate
  • Aboriginal or Torres Strait Islander descent (An applicant's race is a genuine occupational qualification and is authorised under section 14(d) of the Anti-Discrimination Act 1977).
  • Certificate 111 in Aboriginal & Torres Strait Islander Primary Health Care or currently enrolled or has relevant Aboriginal health qualification and a willingness to undertake further training as identified.
  • Experience and demonstrated understanding of Aboriginal & Torres Strait Islander health needs and ability to initiate effective strategies when working with Aboriginal and Torres Strait Islander people.
  • Current NSW Drivers Licence (P1 and P2 acceptable) 

This is an Identified Aboriginal/Torres Strait Islander Position. Applicants for this position must be of Aboriginal descent through parentage, identification as being Aboriginal and being accepted in the community as such. Exemption is claimed under Section 14 of the Anti Discrimination Act 1977.
