Hours Per Week: 38 (Job Share may be considered for part time hours)

Where you'll be working 
Western Sydney Local Health District (WSLHD) is responsible for providing primary and secondary health care for people living in the Auburn, Blacktown, and The Hills Shire, Cumberland and Parramatta Local Government Areas (LGAs) and tertiary care to residents of the Greater Western Region. What you'll be doing

  • The Sustaining NSW Health Families (SNF)  home visiting program is offered to families who need extra help in the first two years of their child's life. The CFH nurse works in partnership with the family to help  improve the health, development and wellbeing outcomes for women, children and families through sustained home visiting. The program aims to improve the transition to parenting using a “strength based” approach and evidenced based assessment tools and activities whilst supporting mothers and families with psychosocial and environmental issues.
  • The SNF program is offered across two locations in Western Sydney.  Fulltime staff in both locations have the opportunity work across SNF and universal child and family health nursing clinics.
  • Training and professional development and clinical support will be provided.

  • Qualification: Registered Nurse
  • Registration: Registered Nurse Division 1


Selection Criteria

  1. Current enrolment with Australian Health Practitioner Regulation Agency as Registered Nurse Division 1, Qualification in Child and Family Health Nursing.
  2. Demonstrated post registration experience working with families and young children, including those from diverse cultural and linguistic backgrounds using a family partnership model.
  3. Demonstrated post registration experience using screening tools including Personal Health Record (PHR) schedule of developmental checks, comprehensive health assessment, SAFESTART psychosocial assessment, ASQ and ASQ-SE and an understanding of NCAST PCI, HOME Inventory,
  4. Effective communication and interpersonal skills with a demonstrated ability to use problem solving skills working within a multidisciplinary team approach in the planning, delivery, and coordination of care.
  5. Demonstrated ability to manage a clinical case load and prioritise work demands in a busy clinical setting.
  6. Demonstrated knowledge of evidenced based Child and Family Health nursing practice, including an understanding of The First 2000 Days Framework and the principles of population health and primary health care.
  7. Demonstrated understanding of early intervention prevention, community development with a working knowledge of the Child Wellbeing and Child Protection Interagency Guidelines
  8. Current NSW Class C drivers license and willingness to use for work purposes.
