About the role

Our single goal is provide a service that builds 'healthier rural people and thriving communities', and in this role, you’ll grow long-term relationships with patients, simplifying referral pathways and access to care to help them manage their health within their own communities.

In this position, you will operate within Dubbo Health Service under the Nurse Manager of Integrated Care in the community. You will be responsible for identifying, assessing and coordinating care from the hospital setting for patients over the age of 16 with complex health and social care needs. Working closely with the treating care team across acute and community settings, and with external healthcare providers, you will support the culturally safe, effective and smooth transition of patients moving in and out of Dubbo Health Service.
