About the role:
The Integrated Registered Nurse Care Coordinator is an integral member of the multidisciplinary healthcare team responsible for coordinating patients health and social services in partnership with the clinical care team and will support access services that address their identified needs.

Where you’ll be working:  
Rylstone MPS is a 32 Bed facility encompassing 2 Bed Emergency Department; 3 Bed acute area and 24 Residential Aged Care Beds. Along side this is the Community health centre offering outpatient services to the community.
