About the role
Our single goal is providing a service that builds 'healthier rural people and thriving communities', and in this role, you’ll grow long-term relationships with patients, simplifying referral pathways and access to care to help them manage their health within their own communities.

As an important part of the multidisciplinary care team, you’ll deliver nursing care through a flexible service delivery model, in ambulatory settings and will lead, direct and supervise other care roles in the team, including nurses, students and Aboriginal Health Workers (AHW) to support the delivery of quality care.
You will need to communicate effectively with patients and other health care team members, and importantly be an active participant in fostering our culture and lead by the values of collaboration, openness, respect and empowerment.
