About the role
This position is based with the Acute & Continuing Care Team (ACCT). The team is responsible for providing acute response, assessment, planning and case management for adults living in the community who are experiencing an acute episode and who may require continuing care and case management. The Acute & Continuing Care team provide mental health support for adults (18-64) who are experiencing a mental illness.
The recovery-oriented approach is person centred, trauma informed and involves partnering with general health providers and agencies that can offer clients community living supports such as housing, disability services, vocational, social and educational opportunities.
