About the role
The Disability Pathways Navigator is primarily responsible for providing high level support to the District Manager Disability, to develop processes, pathways and training to assist Western NSW Local Health District staff support people with a disability, their families and carers in their journey through the health care system in accessing appropriate disability care.
This role is also responsible for facilitating clinical and systemic escalations with National Disability Inclusion Insurance Scheme (NDIS), plus identifying and developing appropriate pathways for people with disability with complex needs to access the NDIS or alternate supports, enabling safe and sustainable discharge from hospital and avoid unnecessary admissions.
