About the role
An exciting opportunity to join our Western Virtual Patient Flow Access and Command Centre.

Delivering one service across many places, the Clinical Nurse Specialist 2 will partner with our existing patient flow, inpatient and Connected Care in Community teams supporting discharge coordination processes to meet key performance outcome measures.  

This position will be a rotating roster, morning and afternoon shift, 7 days a week, supporting a coordinated muliti-disciplinary team approach.
