About the role
The Aboriginal Health Practitioner provides a range of services that includes home visits, wound care, chronic disease management.
Provide ongoing support to those in the community accessing Cardiology, Diabetes and Geriatric clinics is required.
An understanding of evidence based, practice patient centred care and demonstrated ability to work effectively both independently and as part of the multi-disciplinary health care team to plan, coordinate, implement and evaluate care for individuals/groups and healthcare programs.
