About the role

An exciting opportunity exists for a Dietitian to join our outpatient chronic and complex care team. We are looking for a dynamic and motivated dietitian with a passion for managing the nutritional requirements of patients with diabetes, respiratory and cardiac conditions and general nutritional concerns in community settings.

You will provide nutritional management of adults with chronic conditions and general nutritional concerns, working as a member of a multidisciplinary team in an outpatient community health setting providing assessment and nutritional management in face to face clinics and via telehealth. 

At Orange Health Service Chronic Care we focus on patient-centred, best-practice care and you will have a full orientation to the service, including clinical supervision and ongoing support from senior dietitians.

The Chronic & Complex Care team is part of the wider primary and community health team. Our dynamic and highly functioning team work as a multidisciplinary team  and collaborate with hospital and community health team and other community based services to promote health and wellbeing and to address patients’ chronic health needs.
