Speech Pathologist Level 3: $54.77 - $56.54 per hour
Psychologist: $37.70 – 55.17 per hour

About the Opportunity

This position is an exciting opportunity to work in the new Disability Navigation team in South Western Sydney Local Health District.

It is temporary backfill for 3 months, with the possibility of further extension.

While principally based at Bankstown Hospital, there is flexibility to work with services across the whole district. Please make contact to find out more before applying.



 What You'll Be Doing

  • Improve the care experience for people with disability, their families and informal supports.
  • Facilitate continuity, coordination and transfer of care between admitted and community settings.
  • Work collaboratively with SWSLHD and external partners to ensure the care needs of people with a disability are met such that fewer acute health responses are needed.
  • Provide guidance, leadership and advice to facilities to support the needs of people with disabilities who present to hospital.
  • Implement proactive care navigation and hospital avoidance strategies for people with disabilities with unmet/unstable care needs.
  • Build and strengthen networks with SWSLHD and external services that support the needs of people with disability.
  • Undertake duties and reporting requirements associated with the position.

How to Apply

  1. Relevant qualifications as a Registered Nurse, Occupational Therapist, Physiotherapist or Psychologist currently registered with the Australian Health Practitioner Regulation Agency (AHPRA)qualified Social Worker eligible for membership with the Australia Association of Social Workers (AASW) or qualified Speech Pathologist eligible for membership with Speech Pathology Australia
    CNC Grade 1:  has at least 5 years full time equivalent post registration experience and in addition who has approved post registration nursing/midwifery qualifications relevant to the field in which he/she is appointed, or such other qualifications or experience deemed appropriate by the employer.
  2. Demonstrated in-depth knowledge of both the health and disability sectors, including the National Disability Insurance Scheme.
  3. Understanding of the principles of access and reasonable adjustment for people with physical, cognitive, communication, psychosocial and sensory disabilities.
  4. Experience coordinating/navigating care for people with complex needs (including where behaviours of concern are present) across multiple services and agencies.
  5. Previous service planning, managerial experience and/or leadership responsibilities.
  6. Previous change management experience.
  7. Demonstrated experience of effective negotiation, collaboration and communication skills with a wide range of stakeholders.
  8. Demonstrated high level computer skills including Word, Excel and SWSLHD databases.
  9. Demonstrated analytical, strategic and problem solving skills
  10. Must possess a valid Australian Drivers Licence (excluding learner driver's licences). If not already in possession, a NSW Drivers Licence must be obtained within 3 months of appointment.
