What you'll be doing
The Stroke Care Coordinator will be responsible for the implementation, coordination and evaluation of stroke care to the patients of Southern NSW Local Health District. This includes provision coordination of the patient’s care through the acute phase of treatment, and the initiation of, and participation in, stroke education and service development across the SNSW Local Health District and external services for example The Canberra Health Service.


Selection Criteria

  1. Current Registration with the Australian Health Practitioners Regulation Agency (AHPRA) as a Registered Nurse with a minimum of 3 years experience in clinical care provisions to persons following stroke.
  2. Demonstrated high level of communication and interpersonal skills, particularly negotiation, with all disciplines to effect change.
  3. Demonstrated leadership skills with the ability to work effectively across organisations or teams simultaneously, to ensure evidence based stroke care is provided to all persons presenting with stroke.
  4. Demonstrated ability to initiate, complete and evaluate quality improvement projects, including the ability to manage and maintain databases and analyse database information.
  5. Proven commitment to ongoing education, including development and delivery of education programs.
  6. Evidence of a current unrestricted drivers’ licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances and overnight stays.



Occupational Assessment, Screening and Vaccination against Specific Diseases
This is a Category ‘A’ position, please read and understand NSW Health policy directive PD2024_015. All new employees must agree to comply with the requirements outlined in this policy. OASV requirements also include proof of vaccination against influenza and is compulsory for NSW Staff that enter any Residential Aged Care Facility (RACF) or who work in NSW Health owned and operated RACFs.
