About the Role

This is a district wide position based across Concord Repatriation General Hospital and Royal Prince Alfred Hospital

This is an innovative and exciting role for an experienced Nurse Practitioner to join a cohesive, energetic and dynamic multidisciplinary service. SLHD RACF Outreach recognises and values the importance of Nurse Practitioners. The Residential Aged Care service seeks to be at the forefront of education and professional development for Nurse Practitioners within Aged Care. The aim is to ensure high quality care occurs within Aged Care Facilities and demonstrate a career development pathway for new nursing graduates interested in Aged Care. 

In addition, this role will focus on the smooth transition of care for patients with severe behavioural and psychological symptoms of dementia, including those entering care from the community and SLHD hospitals. These patients are complex and at high risk of admission with long length of stays. This is a challenging area of practice requiring skill and expertise.  

Clinical:

  • Provide evidence-based extended and advanced nurse practitioner care for patients registered with the SLHD RACF Outreach Service; facilitate and support the transition of inpatients to residential aged care prioritising those experiencing severe behavioural and psychological symptoms of dementia; assist with the management of complex comorbid patients expected to transition to residential aged care in the near future.  

Leadership:

  • Provide clinical leadership and supervision to nursing staff within the SLHD RACF Outreach Service. 
  • Develop and enhance the scope of practice for Transitional Nurse Practitioners and Nurse Practitioners working within the SLHD RACF Outreach Service.
  • Work in conjunction with the Nurse Unit Manager and the Medical Director to ensure all nursing staff are supported to work at their full scope of practice and access professional development opportunities.
  • Support NSW Health and Commonwealth initiatives in relation to Nurse Practitioner roles within Aged Care Facilities.

Education:

  • Develop and implement a clinical training program for Transitional Nurse Practitioners within the service.
  • Work in conjunction with the Medical Director and Geriatricians within the SLHD RACF Outreach Service to facilitate supervision for Transitional Nurse Practitioners within the service.
  • Work in conjunction with the Nurse Unit Manager and CNC to support education opportunities for all nursing staff within the service.

Ideal Candidate
  • Current registration with AHPRA and authorised to practice as a Nurse Practitioner by the NSW Nurses & Midwifery Registration Board (AHPRA) or working towards such a qualification. If transitional, 7 years full time equivalent post registration experience, with at least 5 years full time equivalent clinical experience in the specialty of the Nurse Practitioner role.
  • Demonstrated ability to perform in a clinical leadership role evidenced by participation in practice development activities including mentoring and education.
  • Prior experience within Residential Aged Care Facilities preferred.
  • A current NSW Drivers licence (P1 and P2) accepted.
