You will come in to work at Canterbury Community Health 

Community Mental Health Services provide a range of services designed to help support the recovery and help consumer stay well in the community. The team provide specialist services for young people, children and families, adults, and older people. Community mental health services include a specialist Aboriginal Mental Health Team, bilingual mental health workers and Peer Support Workers. Our main sites are at Canterbury, Croydon, Camperdown, Marrickville, and Redfern Community Health Services. The role will be based at Canterbury Community Health. 
  
The Acute Care Service model of care provides:

  1. Mental Health assessments to people experiencing acute mental health problems ( including a rapid response when indicated) and determining an appropriate pathway to care (eg community treatment; assist consumers access  and admission to inpatient treatment)
  2. Short term intensive treatment in the community often as an alternative to hospital 
  3. In reach to hospital to promote effective follow up post mental health admission 
  4. Gatekeeping- developing opportunities for assessment and community treatment at key points (eg; Assessment Unit and Admission Office) 
  5. The model provides short term treatment after assessment up until the acute symptoms are resolved or the consumer is effectively transferred and engaged with an appropriate treatment provider. 
  
The clinician will practice in keeping with the standardised Sydney LHD position description for the profession in addition to this position description.

Ideal candidate
  • Holds recognised tertiary qualifications appropriate to profession. Full registration and eligibility for membership to professional association, as appropriate.
  • Demonstrated ability to develop and maintain effective relationships with GPs, Service partners and external providers.
  • Has the ability to work on a 7 day rotating roster. 
  • Holds a current NSW Drivers Licence (P1 and P2 acceptable).
