About the Role
Community Mental Health Service is committed to the provision of quality health services to the mental health community. We aim to deliver mental health services which, through research and innovative commitments, set standards that are emulated by other national and international organisations. We aim to lead through achievement and innovation in clinical practice, research, teaching, training and management. We are committed to developing the highest skills in our community mental health staff through different educational programs.
 
The Community Mental Health/Core Team model of care states:
 
The "Community Mental Health Team" (Core Team) is the mainstay of the community mental health system. Core Teams and provide a range of clinical services including intake, assessment, consultation, treatment ,and care coordination. Care coordination is the main component of this model of care and includes extended care coordination, targeted packages of care, and short term care coordination.
 

Consumers under the care of Core Teams will either have:

  1. Short term interventions and may be discharged or referred back to their GPs or other treatment providers after a period of weeks or months.
  2. A period of treatment as recommended by evidence based practices.
  3. Ongoing care and monitoring for prolonged periods.

Ideal Candidate

  • Current nursing registration with AHPRA.
  • Previous mental health experience desirable.
  • Demonstrated competencies to provide contemporary mental health practice.
  • Current NSW Drivers Licence (P1 and P2 acceptable).
