Our mission is to provide excellence in social work input to our patients, their carers and family. We provide a social work service to all clinical areas at Sutherland Hospital including psychosocial assessment and support, brief crisis counselling, carer support and discharge planning. We also provide an After Hours Crisis On Call Social Work Service to the hospital – 24 hours, 7 days per week.

This recruitment is for a permanent full time level 1-2 Social Worker. An eligibility list will also be created for future permanent and temporary full time or part time positions.

What you'll be doing
Social Work Level 1-2 clinicians can rotate across clinical areas – the successful applicant will have a clinical allocation of Oncology (inpatient and outpatient clinic), Aged Care or another clinical area dependent on need. Shifts will predominantly be Monday to Friday, however some shifts will be expected on the weekend on a roster basis. A supportive learning environment is provided with clinical supervision, professional development and team working.

On a day to day basis hospital Social Workers may:
• Assess a patient’s situation and develop interventions based on that assessment to improve patient outcomes
• Conduct counselling and targeted therapeutic interventions
• Educate patient/family/carers on health care being provided and improve communication between them and healthcare workers
• Work as part of multidisciplinary teams to assist in patient care, improved treatment outcomes and successful return home after a hospital stay
• Case management and the coordination of services both within and external to the hospital
Social Workers plan, coordinate and deliver high quality Social Work care to patients, families and carers consistent with South Eastern Sydney Local Health District (SESLHD) policies, procedures and standards.

Selection Criteria


• Qualifications in accordance with Schedule C of the NSW Health Service Health Professionals (State) Award providing eligibility for membership of the Australian Association of Social Workers.
• Demonstrated core competency skills in psychosocial assessments, individual and family casework, crisis intervention, counselling, advocacy and discharge planning.
• A good knowledge of community resources and experience liaising with community organisations.
• Demonstrated high level written and verbal communication, and interpersonal skills, and the ability to proactively engage with patients/clients to enhance service delivery.
• Ability to work as part of a multi-disciplinary team and to exercise independent professional judgement on routine matters, commensurate with years of experience
• Computer skills and the capacity to use health information systems.
• Demonstrated commitment to quality improvement and evidence based practice and a commitment to ongoing clinical and professional learning
• Understanding of crisis interventions and the ability to participate in the After Hours Crisis On-Call Social Work Service
