Prince of Wales Hospital - Clinical Nurse Specialist Grade 2- Rapid Response Palliative Care
Rapid Response in Palliative Care plays a critical role in ensuring timely, person-centered care to manage complex symptoms and transition palliative and end of life care from hospital to community. We are seeking a skilled and compassionate Clinical Nurse Specialist to help develop our service.
Where You'll Be Working:
Prince of Wales Hospital and Community Health Service is a Level 6 Tertiary Referral Hospital with an inpatient bed base of 370. Each year we care for more than 70,000 patients in our Emergency Department and have around 50,000 admissions to the inpatient units. POWH offers an outpatient service and rural outreach service and provides more than 900,000 occasions of non-admitted patient care each year, including innovative virtual models of care. We have transitioned the majority of inpatient units and wards to the Acute Services Building (ASB) including a state of the art Emergency Department and Intensive Care Unit. What You'll Be Doing:
The Nursing and Midwifery – Our Strategy for Transforming Person Centred Cultures 2024-2029 guides our profession in the development of effective person-centred practice and healthful cultures within SESLHD. We advocate for shared decision-making, collaborative relationships, compassionate and person-centred leadership, and supporting innovative practices.
The Prince of Wales hospital Palliative Care services deliver person-centred palliative care to our patients and their family within the hospital setting or who are transitioning to the community palliative care team. As the Rapid Response Clinical Nurse Specialist Grade 2 in SESLHD you will provide individualised, case managed complex patient/client care and leadership within the Palliative care service to maximise the health outcomes for the patient/client. As a Clinical Nurse Grade 2 in SESLHD you will be recognised as a healthcare professional employing your clinical skills and critical thinking in delivering high quality patient care. You will exercise extended autonomy of decision making and professional knowledge and judgement in providing complex care requiring advanced clinical skills.
As a Clinical Nurse Grade 2 You Will:
- Provide primary case management of a continuum of specialty care involving both inpatient and community-based services.
- You will be an integral part of the interdisciplinary team in the provision of person-centred care; and lead, support and supervise other nurses and care roles. You will strive to ensure that the people who receive healthcare experience exceptional care that is person-centred, safe, high quality and compassionate.
- You will be part of a team that strives to support each other to reach our potential, focus on what matters most and continue to be innovative in approaches to practice.
- As the Rapid Response Clinical Nurse Grade 2 in Palliative Care at the Prince of Wales Hospital will be encouraged to share your clinical expertise and further develop your practice with the professional development opportunities available.

Selection Criteria:
- Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse/ Midwife and current NSW Driver’s License and willingness to travel as required in role
- Relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate.
- Demonstrated ability to manage complex clinical care and service delivery issues and problem solve through the development and use of clinical management plans.
- Demonstrated ability to exercise extended autonomy of decision making while working as part of a multidisciplinary team and apply professional and ethical boundaries when dealing with complex situations.
- Demonstrated skills in the development and facilitation of patient and carer education; and ability to mentor and coach nursing staff to enhance professional development.
- Demonstrated high level written and verbal communication, interpersonal skills and the ability to identify key messages, issues and concerns when communicating with others.
- Demonstrated experience in leading and initiating activities to improve clinical service delivery and patient clinical outcomes.
- Demonstrated competence in the use of health-related information technology.
