  1. Tertiary qualification in social work and eligible for registration membership with AASW.

  2. A demonstrated high level of communication will be required to contribute to Trapeze service delivery, resource development and clinical support.

  3. Demonstrated postgraduate experience and knowledge of adolescents living with chronic conditions, the developmental considerations involved and associated transition to adult healthcare challenges.

  4. Excellent skills to work in partnership with a multidisciplinary team and other service providers.

  5. Experience in care coordination and knowledge of health care and psychosocial resources in the community

  6. Competence in computer skills and the use of electronic medical record.

  7. A Current NSW Drivers Licence

A COVID-19 Vaccine Medical Contraindication form must be completed by a GP or Medical Practitioner.

An exemption to the Public Health Order requirements will only be considered if you are unable, due to a medical contraindication, to be vaccinated against COVID-19.
