Trapeze is a multi-disciplinary team responsible for providing and facilitating care coordination to support transition to adult services for young people aged 14-25 years with chronic conditions and their families/carers who are known to The Sydney Children’s Hospitals Network (SCHN). This is an exciting opportunity for a suitably qualified Social Worker to work with a talented multidisciplinary team in providing support for young people whilst they transition. This positon is up to 32 hours a week maternity backfill for 12 months. The successful applicant will work across the Sydney Children's Hospital Randwick and the Children's Hospital at Westmead. Applicants seeking to job share and work at one site will also be considered. 

What you will be doing
The Social Worker is an integral member of the multidisciplinary team and will:
•Provide psychosocial and care coordination interventions to improve the transition process for these young people and their families. 
•Conduct comprehensive transition assessments, provide individual and family support and collaborate with the multidisciplinary Trapeze team, other hospital subspecialty teams and external stakeholders. 
•Support young people with high risk clinical conditions to optimise health outcomes by developing their capacity for self-management as developmentally appropriate.
