What you'll be doing
The case manager will provide case management to children with complex physical and cognitive disabilities, who are clients of Kids Rehab. The case manager, in collaboration with the treating team, develops and implements rehabilitation plans and undertakes outreach liaison with families, schools, community based clinicians and services.

Participation in teaching, research and quality assurance activities within Kids Rehab and the SCHN is essential.
