Where you'll be working
Northern Sydney Local Health District (NSLHD) - St Leonards

What you'll be doing
The Clinicians/Case Workers in the Assertive Community Management (ACM) team will provide assertive outreach case management for clients using drugs and/or alcohol; including referral to inpatient/outpatient drug and alcohol treatment, undertaking home visits, transporting clients to/from medical appointments, housing and income support, NDIS applications, referrals for community-based supports, liaison with medical teams and case coordination.


People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria:



  1. Current AHPRA registration for relevant discipline - in Psychology, Occupational Therapy or Counsellor with current registration in Counsellors Association of Australia (PACFA), or eligibility for Membership for Social Workers with the AASW. For Health Education Officer Graduate relevant tertiary qualifications in health promotion, public health or other behavioural health sciences.
  2. Demonstrated experience and commitment to professional and ethical boundaries in consultation with a medically led multi-disciplinary treatment team.
  3. Knowledge and understanding of the social and emotional needs of patients with drug and alcohol issues including clinical response and management of trauma, grief and loss, domestic and family violence and child protection.
