Where you'll be working

Ryde Hospital - CNC Team, located within the Emergency Department

Ryde Hospital is having an incredible $479 million complete hospital redevelopment that will transform the delivery of healthcare for the Ryde local community.
The Ryde Hospital Redevelopment will result in new health facilities and enhanced services including emergency, ICU/critical care, inpatient units, outpatient/ambulatory care services and much more.
Construction commenced in early 2023, and will provide you with a wonderful and exciting opportunity to be part of the Ryde team to change the future health services for the Ryde Community completely.


What you'll be doing

The role is responsible for delivering specialised assessment, care coordination, and discharge planning for older people presenting to the Emergency Department. Working collaboratively with ED clinicians, the position focuses on improving health outcomes and reducing unnecessary hospital admissions and representations for individuals aged 70 and over, and Aboriginal people aged 50 and over. The role ensures timely, person-centred care for frail older adults with complex needs, supporting safe discharge home with appropriate services or coordinated admission to hospital with a care plan in place.


People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.

Candidates will need to meet the following criteria: 

  1. Registered Nurse who has a minimum of 5 years full time post graduate experience. In addition, the applicant must have an approved, or working towards post graduate qualification and/or substantial relevant experience to the specialty.
  2. Excellent written and verbal communication and interpersonal skills, including the ability to work with people from diverse backgrounds and people from Government and Non-government agencies.
  3. Demonstrated ability to work effectively both as a member of a multidisciplinary team and independently.
  4. Demonstrated computer skills with experience using MS Office programs and electronic filing systems.
