Community Registered Nurse
Hospital in the Home (HITH) provides acute care in the community with the aim to avoid hospitalisations within the NSLHD. HITH utilising senior multi-disciplinary clinicians to provide a holistic approach. This senior RN role is professionally satisfying and you will be rewarded as our patients just love this modality of care.

What you'll be doing
Assess, plan, implement and evaluate community nursing care, in the patient home or clinic, in collaboration with individual/s and the greater health care team to achieve quality patient outcomes.
People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.
Candidates will need to meet the following criteria:
- Registered Nurse –Division 1 with Australian Health Practitioner Regulation Agency (AHPRA).
- Demonstrated ability to communicate effectively with patients, carers and a diverse range of health and non-health care providers.
- Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, patient safety, quality of care and team cohesion.
- Sound experience using electronic medical record systems and office applications, and evidence of personal time management capabilities and effective negotiation skills.
