Clinical Nurse Specialist - Chronic and Complex Care Coordinator

• Registered Nurse (Division 1), with current AHPRA registration with relevant post-registration qualifications and at least 3 years experience working in Complex Care Coordination of their specified post-graduate qualification and a current NSW driver’s licence.
• Substantial and current experience in Complex Care Coordination, using advanced clinical skills in patient assessment and care planning.
• Demonstrated ability to communicate effectively with patients, carers, and a diverse range of health care providers including those in government, non-government, residential aged care and community organisations.
• Demonstrated ability to work well within a multidisciplinary team, with an emphasis on service delivery, quality of care and team cohesion.
• Sound experience using Microsoft Office applications and evidence of personal time management capabilities and effective negotiation skills.
• Experience in practice that promotes self-management support, behaviour change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.
• Ability to identify and improve opportunities to enhance continuity of care between acute hospitals, primary care providers and community based services and a comprehensive knowledge of primary health and community based services.
